COMFORTage II – Digital innovations tackling dementia and frailty in the old age
This session will present how the COMFORTage project moves beyond fragmented digital tools to deliver an integrated, data-driven ecosystem for dementia and frailty care. The session will introduce the project vision and the emerging Ageing Data Space, highlighting its alignment with the European Health Data Space (EHDS) and the practical realities of interoperability and regulatory compliance. Insights from large-scale pilots and real-world deployments will demonstrate how innovation can reshape everyday clinical routines — not just conceptually, but in daily dementia and frailty management. Live technology demonstrations will showcase user-centred solutions specifically designed to be accessible, acceptable, trustworthy and meaningful for older adults and healthcare practitioners. The concluding round table will address critical success factors, regulatory friction points, EHDS readiness, and the future vision of COMFORTage to successfully transform the Quality of Life of older adults.
